B. Todd Schaeffer, M.D., F.A.C.S. is a board certified surgeon who specializes in endoscopic surgery of the nose, face, neck, sinuses and skull base. He has been performing endoscopic and laser surgery for over twenty years since graduating from the Mount Sinai School of Medicine. Dr Schaeffer is the Chief and Associate Chairman of the Department of Otolaryngology -Communicative Disorders at North Shore University Hospital at Manhasset. He maintains his private practice offices in Lake Success. He is co-director of NY Facial MediSpa and The NY Surgical Facility, LLP (Joint Commission accredited) located in his offices. He has a dual teaching position as an Assistant Clinical Professor of Otolaryngology-Head & Neck Surgery at The Mount Sinai School of Medicine and The New York University School of Medicine. His teaching efforts have helped train our next generation of facial plastic surgeons and endoscopic head and neck surgeons. Dr Schaeffer's clinical interests are endoscopic surgery in the Head & Neck region. He performs concurrent cosmetic and functional nasal and sinus surgery, which make him uniquely qualified from his peers. Rhinoplasty and septoplasty are two commonly performed procedures in his practice. He uses closed endoscopic techniques, which are less invasive (no packing is required), more precise due to better visualization and quicker healing due to fewer incisions. Over thirty percent of his Rhinology practice is revision cases. These are patients who had their initial surgery with another surgeon and seek Dr. Schaeffer’s expertise to further improve their outcome.

Endoscopic Liposhaving of the neck is an original technique described, published and employed by Dr. Schaeffer to recontour and sculpt the neck. (Archives of Facial Plastic Surgery 2000). For those patients in Dr Schaeffer's practice who are not ready for facial enhancement with plastic surgery, other minimally invasive procedures are offered. These include office procedures such as Botox, Restylane, Juvederm and Radiesse, which help restore volume and youthfulness to the face immediately with no down time.

Dr. Schaeffer has been at the forefront of minimally invasive procedures also in sinus and skull base surgery by utilizing new technology and techniques. Dr Schaeffer and a colleague from North Shore University Hospital in Manhasset were the first to perform Balloon Sinuplasty in New York State in 2006. At the time, only thirty surgeons were credentialed world wide to perform this minimally invasive procedure. Balloon Sinuplasty opens blocked paranasal sinuses with a small balloon placed in the natural blocked sinus opening. This procedure offers quick recovery, little to no bleeding and no tissue removal. This procedure is quickly becoming a part of the armamentarium of endoscopic sinus surgeons with Dr. Schaeffer being one of the most experienced in this procedure. Also since 2006, Dr. Schaeffer has partnered with Neurosurgery colleague Dr. Mark Eisenberg also of North Shore and LIJ. Dr Schaeffer performs the approach and access for removal of skull base and brain tumors to be removed through the nose. This endoscopic technique is most commonly used to remove pituitary tumors. This approach offers a more complete tumor removal and an easier recovery for the patient since no nasal packing is required. This endoscopic technique is only available in limited major health institutions in the country. Besides pituitary tumors, other skull base lesions include craniopharyngiomas, Rathke’s Cleft Cysts, chordomas, chondrosarcomas, meningiomas, CSF leaks, meningoceles, encephaloceles, esthesioneuroblastoma, petrous apex lesions and osteomas can selectively be removed through the nose with this minimally invasive approach. Recently, Dr Schaeffer’s office was designated a clinical research site for a new minimally invasive balloon office procedure for the treatment of chronic sinusitis. This procedure, called FinESS, avoids general anesthesia, is performed in the office and treats the most commonly infected sinuses by opening the bottleneck area of obstruction. Using an ultra slim endoscope of 0.5mm, the blocked sinus opening is dilated using a balloon catheter under direct endoscopic view.

This year, 2010 has been a busy year for Dr. Schaeffer. In January 2010, Dr. Schaeffer was co-director for the inaugural course “Endoscopic Transnasal Approaches to Pituitary Tumors and the Skull Base” held at the new North Shore-LIJ Bioskill Laboratory. In April 2010, he was invited as a guest lecturer and surgeon at an international nasal and sinus surgical conference in India. He gave lectures on these new minimally invasive nasal and sinus techniques he has helped spearhead. He also gave surgical demonstration of Balloon Sinuplasty and repair of bilateral choanal atresia to more than 200 attendees. On April 29th Dr Schaeffer was a contributing author for a paper presentation on FinESS at a meeting in Las Vegas. Also, in Philadelphia, on the same date a poster presentation on “Intra-Operative CT scanner in an Endoscopic Endonasal Odontoidectomy” was presented at the American Association of Neurological Surgeons. In May 2010, Drs Schaeffer and Eisenberg performed the first 3D endoscopic removal of a pituitary tumor on Long Island.. In October, Dr Schaeffer will present at a major meeting in Boston a course titled “Minimally invasive Sinus Surgery in the Office under Local Anesthesia.” Later in October he will co-direct with Dr. Mark Eisenberg an Endoscopic Pituitary surgery course “Concepts of Endoscopic Treatment of Pituitary Tumors for the Non-surgeon.”

Dr. Schaeffer has also been involved in local and national organizational affairs by promoting his profession and advocating for improved patient care over the past nine years. Locally, he was the President of the Long Island Society of Otolaryngology-Head and Neck Surgery. Nationally, he is involved as a committee member in legislative, public relations and carrier relations committees for several different Academies. Moshe Ephrat, MD, FACS

Dr. Moshe Ephrat is a fellowship trained and Board Certified surgeon who has expertise and interests in facial plastic and reconstructive surgery as well as all aspects of ear, nose and throat surgery. He graduated medical school from Albert Einstein College of Medicine in New York. Dr. Ephrat completed his residency in Otolaryngology-Head and Neck Surgery at Long Island Jewish Medical Center of Albert Einstein College of Medicine. In addition, he was awarded a fellowship devoted to facial plastic and reconstructive surgery with Dr. Fred J. Stucker, in Louisiana.
